Successful transition plans are student-driven, and high expectations help build motivation toward what’s possible. Transition planning activities will help students articulate their strengths, preferences, interests, and needs to help them envision a pathway to achieve their goals.
Transition Opportunity Partners will provide you with a deliverable in the form of an Individualized Transition Program. This will include a visual timeline of the steps necessary to accomplish academic and personal goals as students transition to postsecondary endeavors and young adulthood. We recommend beginning transition programming as early as possible in high school to help provide the most opportunity to build the appropriate skills and ensure the best possible academic and vocational supports are in place.
The program includes career and interest inventories and student and parent interviews to help guide the creation of a formalized plan.
Our program includes a review of the student’s current Individualized Education Plan (IEP) or 504 Plan to help you advocate for your student’s needs. The review will include current goals and objectives, as well as current accommodations and modifications, providing suggestions for aligning with the student’s transition plan.
The program includes a review of the current course of study and suggestions plans to align with postsecondary education or vocational goals.
A review of existing resources and identification of opportunities that are available at the federal and state level to help facilitate student success in transition.
In addition to career interest inventories, student programs can include opportunities to get additional experience and exposure to careers including club involvement, extracurricular activities, summer internships, and volunteer opportunities.
The program includes tools to help improve students’ understanding of their rights under the Americans with Disabilities Act (ADA) and how this translates to postsecondary endeavors in higher education or employment.
