Individualized Transition PROGRAM PACKAGES

  • Transition Assessments: The program includes career and interest inventories and student and parent interviews to help guide the creation of a formalized plan.


  • IEP Review: Our program includes a review of the student’s current Individualized Education Plan (IEP) or 504 Plan to help you advocate for your student’s needs. The review will include current goals and objectives, as well as current accommodations and modifications, providing suggestions for aligning with the student’s transition plan. 


  • Course Selection Review: The program includes a review of the current course of study and suggestions to align with postsecondary education or vocational goals.


  • Transition Services Planning: A review of existing resources and identification of opportunities that are available at the federal and state level to help facilitate student success in transition.


  • Career Exploration: In addition to career interest inventories student programs can include opportunities to get additional experience and exposure to careers including club involvement, extracurricular activities, summer internships, and volunteer opportunities.


  • Self-Advocacy: The program includes tools to help improve students’ understanding of their rights under the Americans with Disabilities Act (ADA) and how this translates to postsecondary endeavors in higher education or employment.

TRANSITION PROGRAM PLUS $3,375

  • For students in their Senior year of high school, the Transition Program Plus includes the components of the Individualized Transition Plan (left) in addition to 3 additional hours of 1:1 support with your student to either navigate the college application process or to prepare for entering the workforce.


  • College Applications: Assistance with strategy and college selection, guidance through the application process, review of essays, and interview preparation.


  • Job Applications: Evaluation of job skills and readiness plan, resume development, and interview preparation.

PACKAGE AND CONSULTING RATES

Individualized Transition Program - $2,500 for 10 hours ($250/hour)


Individualized Transition Program Plus - $3,375 for 15 hours ($225/hour)


* An additional five hours of consulting are available at the hourly rate of the purchased package if used within one calendar year of completion of package services.


** For individual requests or questions, specialized consulting is available at the ad hoc rate of $350/hour
