Transition Opportunity Partners provides expertise in special education, higher education, and human resources, helping students and their families navigate the transition from high school to postsecondary education and vocational endeavors. We work collaboratively with students and families to incorporate their feedback and help design a transition program incorporating and expanding upon existing plans, supports, and services to help your student reach their individual goals.
Transition planning is mandated under the Individuals with Disabilities in Education Act (IDEA) as part of your student’s Individualized Education Program (IEP). Transition plans are required to include student input into strengths, preferences, interests, and needs and to present a course of study and contacts to support agencies. However, many transition plans are only addressed during the student’s annual meeting and without the continued and dedicated support of a transition planning partner. By creating a transition program for your student, we help increase the likelihood of meeting postsecondary educational and vocational goals through 1:1 personalized activities, services, and supports to guide your student through high school and beyond.
Students with disabilities served under Section 504 do not currently have the same set of benefits and services achieved through transition planning activities. Transition Opportunity Partners uniquely provides these same services and programs to students with 504 Plans who are not currently benefiting from the activities and structures of the transition planning process.

We offer a complimentary 20-minute service with the student and family to discuss individual goals and an overview of the individualized transition program packages. After this initial consultation, we follow up with the family to recommend an appropriate service for their needs.

An educational transition consultant assists students and their parents in planning for postsecondary educational and vocational activities, ensuring that plans are tailored to the individual needs and goals of the student.
While we can step in to help a family at any point in their planning journey, the best time to contact us is in your student's freshman or sophomore year.
No one can ethically offer this guarantee. However, we can work with your student to shape their activities and application to give them the best possible chance.
We review IEP and 504 plans with the parent and student's goals in mind, and we tailor individualized recommendations for goals, services, accommodations, modifications, and transition planning. Our experienced consultants provide advice for you to advocate for the needs of your child, however, we do not attend meetings as representatives or provide legal advice.
